Medical Social Worker provides information related to advanced directives, community. resources, and counseling to assist with financial, emotional, support system and or social concerns They make sure that every patient’s social and emotional needs related to their illness are maintained.
Medical Social Services include:
- Assessing patient/family social, psychological, and emotional needs
- Provide Short Term Counseling
- Provide information and refer to appropriate community agencies
- Assist with solving housing issues
- Provide assistance in case of abuse and neglect
- Assist with long term planning
- Assess finical needs and educate patient regarding health and social benefit eligibility
- Assist with advanced directives and end of life care
